Underwriting Guidelines

The following insurance company guidelines will help you to select your rate classification. Preferred Plus rate classification offers the lowest rates. But it is hardest to qualify for the underwriting requirements. Only a small percentage of the general population qualifies for Preferred Plus. The Preferred classification is easier to qualify for than Preferred Plus and the premiums are still quite attractive. You must meet all of the requirements in the categories that follow to achieve the respective rate classifications for Preferred Plus or Preferred. If you don't meet all of these requirements then choose the Standard classification. Your actual rate classification will be decided by the applicable insurance company. The actual underwriting requirements vary somewhat from one insurance company to the next.

Try to be as complete and forthcoming as possible when evaluating your rate classification from the following medical guidelines. Choosing an unrealistically high classification can yield an unsatisfactory result and lengthen the process.
